Passavant Memorial Homes provides a comprehensive range of residential living and support services to people with intellectual and developmental disabilities who have a primary diagnosis of mental retardation or mental illness.

Community Residential

Passavant provides residential placement in neighborhood homes for children and adults. These homes are staffed 24 hours a day, seven days a week by a caring and compassionate group of professionals dedicated to meeting the needs of the individuals. The supports offered within the home focus on independence, choice and community involvement. Staff ensures that each person adapts to residential living and coordinates the relationships between families, caregivers and supports coordinator personnel.

Life Sharing
In a life sharing setting, individuals live with a family or person  in a private home. Life Sharing offers a stable environment where people can share experiences in a safe, nurturing and caring household. Both children and adults with a variety of needs and challenges can be supported through Life Sharing.

In-Home Waiver
In-home support staff is available to individuals who live within their own family home. The amount of support provided to the consumer by Passavant’s staff is determined by the individuals’ needs along with input from the family.

Respite
Passavant provides temporary care and housing for individuals with intellectual and developmental disabilities within a community home. Respite services are offered to families on a short-term basis when they are traveling, taking vacations or in the event of personal and medical emergencies.

Long-Term Structured Residences
Residential facilities are available to help transition individuals with mental illness into a more independent living environment. The program is designed to teach socialization and independent living skills. Residences offer a highly structured living arrangement for individuals 18 and older who are in need of 24-hour support.

Vocational and Employment Services
pEmployment and vocational training activities focus on developing skills for our consumers that are vital to attaining independence and securing competitive employment. Supports are structured to provide progressive work skills training, pre-vocational skills and work experience. These skills increase the likelihood of successful integration into the local community and workforce.

Supported Living
Passavant provides supports to individuals with a mental health or mental retardation diagnoses who live in their own residences or are in transition to living in the community. The Supported Living goal is to promote self-sufficiency and individuality, and to maximize the individual’s potential for independence. Assistance may include financial management, housing, medication management, health maintenance, transportation, nutrition and daily living skills. Each participant is active in the planning and implementation of their supported living goals.

Seniors Program
An activities-based program is offered for senior citizens with disabilities. Staff members work to identify a plan of activities to help seniors reach their fullest potential and maintain daily life skills.
