Home and Community Based Programs
PMH currently supports over 250 individuals residing in their own homes either independently or with their families. These programs provide services to children and adults with disabilities through Title XIX Waiver programs including Pennsylvania’s Consolidated Waiver and Person/Family Directed Support Waiver Program. Home and Community supports assist the parents by providing Home and Community Based Habilitation, Respite Services, and ISO services. Individuals supported by this program reside in over 15 Pennsylvania counties.
Companion services are provided to individuals living in private residences for the limited purposes of providing supervision and necessary care and minimal assistance that is focused solely on the health and safety of the adult individual (18 and older) with intellectual disabilities.
Direct, face-to-face services provided in home and community settings to assist individuals in acquiring, maintaining, and improving the self-help, domestic, socialization, and adaptive skills necessary to reside successfully in home and community-based settings.
Passavant provides temporary care and housing for individuals with intellectual disabilities within a community home. Respite services are offered to families on a short-term basis when they are traveling, taking vacations, or in the event of personal or medical emergencies.
